Sheet 17 covered CAPEX: how much it costs to buy the device. This sheet enters OPEX (operating expenditure) — the real cost of each session done with it: consumables, staff, maintenance, floor space. It's the figure that actually decides whether a selling price leaves margin, and it's also the one least explained in equipment buying guides: almost all of them talk about the device's price and theoretical return, few about the recurring cost that keeps existing session after session.
Many RF microneedling platforms — the family from Sheet 05, and Morpheus8 itself — run on a single-use disposable tip made by the platform's own manufacturer. That piece's manufacturing cost is low (an estimated $1.50 to $3), but the price the clinic pays for it — always to the manufacturer, with no alternative — is a different story: $100 to $500 per unit, depending on the platform's tier. It's the same logic as the cheap printer and the expensive cartridge, applied to medical devices: the manufacturer's real margin sits in the recurring consumable, not just the initial device sale.

2. Un ejemplo real de cuánto se come el margenA real example of how much margin disappears
An industry guide specializing in med-spa economics gives a concrete example for an RF microneedling session billed at $750: the proprietary disposable tip costs $350 — 46.6% of that single session's billing — and staff account for another 15% to 20% (112-150 dollars). Between the consumable and staff alone, more than 60% of the billing is already gone, before counting rent, marketing, equipment maintenance or CAPEX amortization from Sheet 17.
The pattern is different in injectable treatments: there, the product itself (the hyaluronic acid or biostimulator vial) dominates the cost, and the application material — the needle or cannula — represents only 2-3% of billing. These are two completely different margin structures within the same clinic, and it's worth not applying the same rule of thumb to both.
An industry guide describes, as an example, the cost structure of a benchmark med-spa with $2 million in annual billing: total cost of goods sold (COGS) runs around 28% of billing, of which 20% is injectables and 8% the rest of consumables, with a net margin around 20%. It's an illustrative example of a specific market (U.S.) and clinic type, not a figure to transplant directly onto any Spanish clinic — but it gives a realistic sense of what share of billing stays in direct costs before reaching profit.
3. Qué hacer con esto al fijar un precioWhat to do with this when setting a price
The most common mistake when setting rates is looking only at what competitors charge (next sheet's topic) without first calculating one's own real cost: if the consumable for an RF microneedling session costs $350, charging $300 isn't a competitive price, it's selling at a loss on that line item. The minimum calculation before setting any rate is adding up consumable + staff time + a proportional share of rent/maintenance/device amortization, and comparing that figure against the price one plans to charge — not the other way around.
The block's last sheet enters market rates: what the industry actually charges in Spain for each treatment type, with the same caution about reliable sources as the rest of the project — and how to cross that figure with the CAPEX and OPEX from these two sheets to find out whether a price actually leaves margin.
A session's real cost isn't just the device (CAPEX): proprietary consumables, staff, maintenance and floor space get paid session after session, and often weigh more than equipment amortization.
In treatments with a proprietary consumable (RF microneedling-type), that consumable can represent close to half of a session's billing — very different from injectables, where application material barely weighs 2-3%.
A real reference figure (not universal): in a benchmark med-spa, cost of goods sold runs around 28% of billing and net margin around 20% — useful for comparison, not for copying without adjusting to one's own clinic.
Setting a rate by looking only at competitors, without first calculating one's own real cost, is the most common way to sell at a loss without realizing it.
