Pocos activos cosméticos tienen tanta evidencia clínica acumulada como los retinoides, y pocos se explican con tanta confusión: retinol y tretinoína no son «lo mismo pero más fuerte», son dos moléculas distintas en el mismo camino metabólico, con estatus legal distinto en España. Few cosmetic actives have as much accumulated clinical evidence as retinoids, and few are explained with as much confusion: retinol and tretinoin aren't "the same thing but stronger" — they're two different molecules on the same metabolic pathway, with different legal status in Spain.

1. Un mismo camino metabólico, dos puntos de entrada distintosOne metabolic pathway, two different entry points

La tretinoína es ácido retinoico: la forma ya activa que se une directamente al receptor celular y actúa sin pasos intermedios, lo que explica su efecto más rápido y también su mayor irritación inicial. El retinol, en cambio, necesita que la propia piel lo convierta primero en retinaldehído y después en ácido retinoico — dos conversiones enzimáticas que diluyen su potencia final pero también su irritación, y que explican por qué sus resultados tardan más en notarse. Tretinoin is retinoic acid: the already-active form that binds directly to the cell receptor and acts with no intermediate steps, which explains both its faster effect and its greater initial irritation. Retinol, on the other hand, needs the skin itself to convert it first into retinaldehyde and then into retinoic acid — two enzymatic conversions that dilute its final potency but also its irritation, and explain why its results take longer to show.

2. Qué dice la evidencia, con cifras realesWhat the evidence says, with real numbers

Un meta-análisis en red de 2025 reunió 23 ensayos clínicos aleatorizados con 3.905 participantes comparando isotretinoína, tretinoína, retinol, tazaroteno, ácido glicólico, adapaleno y bakuchiol frente a placebo para el fotoenvejecimiento facial. Para arrugas finas, el retinol mostró un efecto considerable (OR 14,10), por detrás de la isotretinoína oral y por delante de la propia tretinoína tópica (OR 6,87) — un resultado que no encaja con el relato de «el retinol es la versión floja». Para hiperpigmentación, la tretinoína fue superior; para textura y rugosidad, el ácido glicólico. La tretinoína, además, mostró el perfil de seguridad más favorable de todo el grupo, con menos efectos adversos que el tazaroteno o el propio glicólico. A 2025 network meta-analysis pooled 23 randomized controlled trials with 3,905 participants comparing isotretinoin, tretinoin, retinol, tazarotene, glycolic acid, adapalene and bakuchiol against placebo for facial photoaging. For fine wrinkles, retinol showed a considerable effect (OR 14.10), behind oral isotretinoin and ahead of topical tretinoin itself (OR 6.87) — a result that doesn't fit the narrative of "retinol is the weak version." For hyperpigmentation, tretinoin was superior; for texture and roughness, glycolic acid. Tretinoin also showed the most favorable safety profile of the whole group, with fewer adverse effects than tazarotene or glycolic acid itself.

Un ensayo clínico aleatorizado y doble ciego, con diseño split-face en 72 participantes con fotodaño moderado-severo, comparó directamente retinol al 0,25/0,5/1,0% frente a tretinoína al 0,025/0,05/0,1% durante 12 semanas: ambos redujeron de forma significativa arrugas, tono y pigmentación frente al valor basal, sin diferencias estadísticamente significativas entre ambos activos en ninguna de las tres concentraciones comparadas. Los propios autores concluyen que una proporción de diez a uno de retinol frente a tretinoína puede dar un efecto clínico comparable — un dato útil para leer con criterio cualquier etiqueta que compare ambos activos. A randomized, double-blind, split-face trial in 72 participants with moderate-to-severe photodamage directly compared retinol at 0.25/0.5/1.0% against tretinoin at 0.025/0.05/0.1% over 12 weeks: both significantly reduced wrinkles, tone and pigmentation from baseline, with no statistically significant differences between the two actives at any of the three concentrations compared. The authors themselves conclude that a ten-to-one ratio of retinol to tretinoin can give a comparable clinical effect — a useful data point for reading any label that compares the two actives critically.

Novedad regulatoria real: la UE ha limitado el retinol en cosmética desde noviembre de 2025Real regulatory update: the EU has capped retinol in cosmetics since November 2025

El Reglamento (UE) 2024/996 fija, por primera vez, límites máximos de concentración para retinol y sus ésteres (retinil acetato, retinil palmitato) en cosmética: 0,3% de equivalente de retinol en productos faciales de uso habitual, y solo 0,05% en lociones corporales de aplicación extensa. La norma es de aplicación obligatoria para productos puestos en el mercado desde el 1 de noviembre de 2025, con un plazo hasta el 1 de mayo de 2027 para retirar el stock que no cumpla. Desde noviembre de 2025, cualquier cosmético con estas formas de vitamina A debe incluir la advertencia «Contiene vitamina A. Ten en cuenta tu ingesta diaria antes de usarlo» — el motivo, según el dictamen científico en que se basa la norma, es que hasta un 5% de la población podría acercarse al límite de ingesta diaria segura de vitamina A si suma la de los alimentos, los suplementos y la cosmética. Regulation (EU) 2024/996 sets, for the first time, maximum concentration limits for retinol and its esters (retinyl acetate, retinyl palmitate) in cosmetics: 0.3% retinol equivalent in everyday facial products, and only 0.05% in body lotions applied to large areas. The rule has been mandatory for products placed on the market since November 1, 2025, with a deadline of May 1, 2027 to withdraw non-compliant stock. Since November 2025, any cosmetic with these vitamin A forms must carry the warning "Contains vitamin A. Consider your daily intake before use" — the reason, per the scientific opinion behind the rule, is that up to 5% of the population could approach the safe daily vitamin A intake limit once food, supplements and cosmetics are added together.

3. Estatus legal en España: no son intercambiables sobre el papelLegal status in Spain: not interchangeable on paper

La tretinoína es un medicamento de uso dermatológico que en España requiere siempre receta médica, con evaluación individual de la concentración (habitualmente entre 0,025% y 0,1%), del vehículo y del tipo de piel antes de prescribirla. El retinol, en cambio, es un ingrediente cosmético de venta libre, ahora sujeto a los límites de concentración de la nueva normativa europea, pero sin necesidad de supervisión médica para adquirirlo. Esta diferencia legal, no solo la de potencia, es la que determina qué puede ofrecer o recomendar cada tipo de profesional del sector. Tretinoin is a dermatological medication that in Spain always requires a prescription, with individual assessment of concentration (typically between 0.025% and 0.1%), vehicle and skin type before prescribing. Retinol, by contrast, is an over-the-counter cosmetic ingredient, now subject to the new European concentration limits, but requiring no medical supervision to buy. This legal difference — not just the potency one — is what determines what each type of professional in the sector can offer or recommend.
