La Ficha 15 terminó con una advertencia: los exosomas no tienen, hoy, ninguna aprobación regulatoria. Los polinucleótidos (PN) y su forma más purificada, el PDRN (polidesoxirribonucleótido), son casi el caso contrario dentro de la misma «estética regenerativa»: una familia con casi tres décadas de historia como fármaco aprobado, antes incluso de llegar a la estética. Sigue siendo, como todo el Bloque 4, acto médico exclusivo en España. Sheet 15 ended with a warning: exosomes have, today, no regulatory approval at all. Polynucleotides (PN) and their more purified form, PDRN (polydeoxyribonucleotide), are almost the opposite case within the same "regenerative aesthetics": a family with nearly three decades of history as an approved drug, before it even reached aesthetics. It remains, like all of Block 4, exclusively a medical act in Spain.

1. De fármaco para úlceras a booster de piel: una historia con fechasFrom ulcer drug to skin booster: a history with dates

El PDRN no nació en estética. La empresa italiana Mastelli lanzó en 1994 Placentex®, una formulación inyectable aprobada como fármaco «antidistrófico» para trastornos de piel y tejido conectivo —cicatrices, atrofia cutánea—, obtenida entonces de placenta humana. El término «PDRN» como sustancia estandarizada se formalizó en la literatura científica en 1996. Por razones de riesgo infeccioso y ética del origen del tejido, la fuente cambió en la década de 2000 a ADN de esperma de salmón y trucha purificado —el origen marino que sigue usándose hoy—, y la investigación clínica se amplió a úlceras del pie diabético, quemaduras, colgajos isquémicos y zonas donantes de injertos cutáneos, con aplicaciones adicionales en ortopedia y gastroenterología. El salto a la estética como categoría de anti-aging llegó después, entre 2010 y 2020, con Corea como mercado central y productos como Rejuran® aprobados hacia 2014. PDRN wasn't born in aesthetics. Italian company Mastelli launched Placentex® in 1994, an injectable formulation approved as an "antidystrophic" drug for skin and connective tissue disorders — scars, skin atrophy — obtained at the time from human placenta. The term "PDRN" as a standardized substance was formalized in the scientific literature in 1996. For reasons of infection risk and tissue-origin ethics, the source shifted in the 2000s to purified salmon and trout sperm DNA — the marine origin still used today — and clinical research expanded to diabetic foot ulcers, burns, ischemic flaps and skin-graft donor sites, with additional applications in orthopedics and gastroenterology. The leap to aesthetics as an anti-aging category came later, between 2010 and 2020, with Korea as the central market and products like Rejuran® approved around 2014.

El mecanismo mejor documentado del PDRN es su acción sobre el receptor de adenosina A2A, que estimula angiogénesis y actividad de los fibroblastos —es el mecanismo detrás de su uso histórico en cicatrización de heridas, y el que se traslada, por extensión, a su uso estético para mejorar calidad de piel. En la aplicación estética actual, los productos más documentados son de la familia PN-HPT (Polynucleotides Highly Purified Technology) del mismo fabricante, Mastelli, comercializados como Plinest® y Newest® —ambos dispositivos médicos de Clase III con marcado CE, dentro del mismo Anexo XVI del Reglamento (UE) 2017/745 que los productos de las Fichas 13 y 14. Los dos estudios son reales y con cifras concretas —no cifras de marketing sin respaldo—, pero comparten las mismas limitaciones: diseño abierto, sin grupo control, y financiación total o parcial del propio fabricante para los costes de publicación (APC). Son datos post-comercialización o de cohorte abierta, no ensayos aleatorizados. Eso no los invalida, pero sitúa a esta familia en un punto intermedio: ni con el peso de un ensayo controlado como el de la CaHA en la Ficha 14, ni sin ningún marco regulatorio como los exosomas de la Ficha 15. Both studies are real, with concrete figures — not unsupported marketing numbers — but they share the same limitations: open-label design, no control group, and full or partial funding from the manufacturer itself for publication costs (APC). This is post-marketing or open-cohort data, not randomized trials. That doesn't invalidate it, but it places this family at a middle point: neither with the weight of a controlled trial like CaHA's in Sheet 14, nor with no regulatory framework at all like the exosomes in Sheet 15.

Un artículo de 2022 sobre PN-HPT en rejuvenecimiento del tercio medio facial, del mismo campo de investigación, fue retractado formalmente por la revista (Journal of Cosmetic Dermatology) en 2023. No ha sido posible verificar el motivo exacto de la retractación a partir de una fuente primaria accesible —el aviso de retractación quedó bloqueado por control de acceso durante la investigación de esta ficha—, así que se señala aquí el hecho en sí, sin especular sobre la causa. Se incluye porque es exactamente el tipo de dato que este proyecto se compromete a no omitir: una retractación es una señal real sobre la calidad del cuerpo de evidencia de una categoría, y forma parte de leer la investigación con el mismo rigor en los datos favorables que en los desfavorables. A 2022 article on PN-HPT in mid-face rejuvenation, from the same research field, was formally retracted by the journal (Journal of Cosmetic Dermatology) in 2023. It hasn't been possible to verify the exact reason for the retraction from an accessible primary source — the retraction notice was blocked by an access control during research for this sheet — so the fact itself is noted here, without speculating on the cause. It's included because it's exactly the kind of data this project commits to not omitting: a retraction is a real signal about the quality of a category's evidence base, and reading research with the same rigor applied to favorable and unfavorable data is part of the job.

3. Riesgos y por qué esta familia no es intercambiable con los exosomasRisks, and why this family isn't interchangeable with exosomes

En los dos estudios reales revisados para esta ficha, el perfil de seguridad reportado fue favorable: eventos adversos transitorios (edema, eritema, dolor, equimosis, induración) resueltos espontáneamente en minutos o, como máximo, unos días, sin efectos adversos graves ni inesperados. Ese dato es real, pero conviene leerlo con la misma cautela que el resto de esta ficha: son estudios sin grupo control, con financiación total o parcial del propio fabricante para los costes de publicación, y sin el peso de un ensayo aleatorizado de mayor tamaño. Dicho esto, la diferencia de fondo con la Ficha 15 es regulatoria, no solo de seguridad percibida: los productos PN-HPT descritos aquí son dispositivos médicos de Clase III con marcado CE, con un fabricante identificable y trazable, mientras que los exosomas cosméticos de la Ficha 15 no cuentan, a día de hoy, con ninguna aprobación regulatoria equivalente. In the two real studies reviewed for this sheet, the reported safety profile was favorable: transient adverse events (swelling, redness, pain, bruising, induration) resolving spontaneously within minutes or, at most, a few days, with no serious or unexpected adverse effects. That's a real finding, but it's worth reading with the same caution as the rest of this sheet: these are studies without a control group, with full or partial manufacturer funding for publication costs, and without the weight of a larger randomized trial. That said, the fundamental difference with Sheet 15 is regulatory, not just perceived safety: the PN-HPT products described here are Class III CE-marked medical devices, with an identifiable, traceable manufacturer, while the cosmetic exosomes in Sheet 15 currently have no equivalent regulatory approval.

Con esta ficha se cierra el Bloque 4 (Inyectables y regenerativos): ácido hialurónico en sus tres técnicas (Ficha 13), bioestimuladores sin ácido hialurónico —PLLA y CaHA— (Ficha 14), exosomas (Ficha 15) y polinucleótidos/PDRN (Ficha 16). El hilo conductor de las cuatro fichas es el mismo: la misma etiqueta comercial —«bioestimulación», «regeneración»— puede esconder niveles de evidencia y de respaldo regulatorio muy distintos entre sí, y solo mirando cada producto por separado se distingue uno de otro. This sheet closes Block 4 (Injectables and regenerative treatments): hyaluronic acid across its three techniques (Sheet 13), biostimulators without hyaluronic acid — PLLA and CaHA — (Sheet 14), exosomes (Sheet 15) and polynucleotides/PDRN (Sheet 16). The thread running through all four sheets is the same: the same commercial label — "biostimulation," "regeneration" — can hide very different levels of evidence and regulatory backing, and only looking at each product separately tells one apart from another.
